• An annual, non-refundable fee of $495 will be required of all patients to cover non-reimbursable services. This can be billed directly to you or paid in person at your next office appointment.

• You will still be responsible for co-payments, co-insurance, and deductibles outlined by your insurance plan.

• Discounts will be provided for multiple family members of the office.

• If this annual fee creates a financial hardship but you wish to remain a patient with us, please contact the office to discuss a payment schedule or fee reduction.

• An additional $75 no-show fee (per occurrence) will still be implemented if an appointment is not changed or cancelled at least 24 hours before your scheduled appointment time. For Monday appointments, cancellations or postponements will be required the Friday before.

• Included in this annual fee will be the following benefits:

  • Maintenance of a secure electronic medical record systemic in order to provide you access to review your chart and results

  • Communication regarding your care with the other physicians and resources within the Cedars-Sinai Medical community in order to optimize your medical care

  • Prescription refills, formulary options, and insurance authorizations and appeals of denials during normal business hours only

  • Completion of work/school excuses, jury duty forms; employment/disability forms less than two (2) pages in length

  • In-office phlebotomy rather than a separate visit to Cedars-Sinai lab or commercial labs

  • Laboratory and blood work maintenance and handling

  • Administration of injectable medications

  • Release of your medical record and communications to other health care providers

  • Medication samples, vouchers, and discount coupons when available and appropriate

  • You will be contacted following your office appointment only for lab result concerns or changes in medication/management

The Enhanced Package includes all of the provisions of the Basic Package plus the following:

• Availability through our CS-Link electronic medical system to communicate with the office staff or myself with responses within 48 hours. Please remember, CS-Link messaging is NOT intended to diagnose or treat any medical conditions.

• After-hours and weekend prescription requests including other inquiries and questions. These requests will be responded to within 48 hours.

• Priority updates and communication on recent labs or study results.

• Office appointments or telemedicine appointments within 1-2 business days to be seen by either a physician assistant or myself.

• Waiver of no-show fee.

• $2500 annual fee per patient, non-refundable and paid on a non-prorated 12 month basis; discounts will be provided for multiple family members.

The Premier Package includes all the of the provisions of the Basic and Enhanced Package as well as the following:

• 24 hours, 7 days a week telemedicine, cell phone, or email availability.

• Direct email and cell phone access to your physician.

• Office appointments same day or next business day to be seen exclusively by your physician.

• Expedited referrals to other medical specialists as indicated including orders for home health and IV therapies- to be arranged by our office staff.

• Fast, convenient labs, imaging, or other required tests (including COVID testing).

• Executive full-body physicals including ultrasounds, echocardiograms, stress testing, lung function testing as indicated.

• Annual executive physicals with the majority of procedures to be completed the same day in our office.

• $5000 annual fee per patient ($250/month), non-refundable and paid on a non-prorated 12 month basis; discounts will be provided for multiple family members.
